About the role


This is an exciting opportunity for an experienced Credentialed Diabetes Nurse Educator with care coordination experience to join our team at HARP.

The Diabetes Nurse Care Coordinator role provides care coordination across the hospital and community continuum ensuring integrated seamless care.

Care is patient centred, based on meeting client's individual needs which can include physical and mental health, psychosocial and environmental needs.

The role targets clients with a focus on complex conditions and or chronic disease.

This includes active participation in the interdisciplinary case conferences for optimal care to the right patient at the right time.

The Diabetes Nurse Care Coordinator manages a clinical case load of clients and assists in intake and clinical prioritisation to facilitate smooth patient flow into the service.

They will also act as a clinical diabetes resource and clinical advisor to the integrated services team and wider Eastern Health service providers.

About you

To be successful in this role you will possess;
Current registration with AHPRA

Post Graduate qualifications in Diabetes Nursing

Credentialed Diabetes Nurse Educator

Highly developed interpersonal and leadership experience

A strong commitment to high quality care

The ability to work as part of a diverse team

Excellent communication and organisational skills

Current Victorian Drivers Licence

Selection Criteria

Essential:
Registration with the Nursing and Midwifery Board of Australian (NMBA) via AHPRA

Essential:
Bachelor of Nursing or other recognised equivalent formal qualification leading to condition free registration in Australia

Essential:
Post Registration or Post Graduate qualification or further study in Diabetes

Essential:
Member of the Australian Diabetes Educators Association (ADEA)


Essential:
ADEA credentialed status


Desirable: 5 years post graduate experience

Desirable:

Extensive experience in the area of clinical expertise with demonstrated ability to work with a diverse and complex professional workforce.
