Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.

About the role


We are seeking a values-driven and proactive Youth Services North/West Metro Case Manager to coordinate all secondary consults for clients of the North West Metro service.

You will successfully

  • Deliver flexible and assertive case management response to clients with professional and consistent case management to those experiencing homelessness or at risk of experiencing homelessness
  • Ensure that young people within the North West region receive an appropriate service response that upholds The Salvation Army's Model of Care, Journey Towards Independence
  • Work collaboratively within Youth Services Integrated Model, having access to all programs. With acknowledgment of the value that an Integrated service delivery model has in supporting sustainable outcomes
  • Hold Interim Response clients via the access point
  • Complete comprehensive intake and risk assessment for all young people, incorporating adequate assessment of the needs of accompanying children where relevant
  • Work to the direction of case management plans in assisting young people to attend appointments relating to health & wellbeing, sustainable housing, education & employment, life & living skills and social inclusion & participation
  • Assist young people with a range of activities in preparing for independent living
  • Participate in duty rosters and actively manage youth entry point


This is an 8 month, full time position, 38 hours per week, based across our Sunshine and North Fitzroy sites.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Level 5.

You will have

  • Tertiary qualification in social sciences, welfare or related field with a degree qualification preferred. Relevant experience in a social service environment
  • A sound understanding of theories relating to trauma and attachment as they relate to service delivery for young people who have suffered neglect and abuse, both on an individual level as well as within the broader refuge environment
  • Understands and identifies indicators of physical, emotional, psychological or sexual abuse in infants, children and adolescents
  • Knowledge of youth homelessness, crisis intervention and the ability to collaborate with other program areas
  • Skills in engaging with the specified client group; demonstrated capacity to build effective relationships with clients based on social work, youth work and psychologically informed environments
  • Demonstrated skills in engaging & working with young people with complex support needs including AOD, mental health, out of home care and adolescent violence
  • Assessment skills and knowledge of a range of theories including attachment, trauma, child and adolescent development and strengthsbased approaches
  • A current VIC Drivers Licence
  • Valid VIC Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
