Care Co-Ordinator (Community Registered Nurse) - Melbourne North

Our Melbourne North office is seeking an experienced and dedicated Care Co-Ordinator to join our dynamic team.

Care Co-Ordinator (Community Registered Nurse) -Part Time


Location:
Melbourne Northern Suburbs and Surrounding Areas


Responsibilities:

As a Care Co-Ordinator, you will play a vital role in providing exceptional care to our clients in the comfort of their homes.

Your responsibilities will include, but are not limited to:


  • Monitor and evaluate the quality of nursing care, identifying areas for improvement and implementing quality assurance initiatives.
  • Participate in continuous quality improvement activities.
  • Conduct postdischarge followup to assess patient progress, address concerns, and prevent readmissions.
  • Generate reports on key performance indicators, client outcomes, and other relevant metrics.
  • Analyze data to identify trends, areas for improvement, and opportunities for optimization.
  • Manage the referral process, including receiving and processing referrals from healthcare professionals or agencies.
  • Ensure timely communication and coordination with referring entities.
  • Maintain accurate and uptodate client records, including care plans, assessments, and communication logs.
  • Ensure compliance with privacy regulations and organizational policies.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to optimize patient outcomes.
  • Participate in interdisciplinary team meetings to discuss and plan patient care.
  • Maintain regular communication with clients and their families to assess satisfaction and address concerns.
  • Communicate with care providers to ensure they are wellinformed about the clients' needs and preferences.
  • Collaborate with healthcare professionals, such as nurses and therapists, to coordinate care plans.

Qualifications:


  • Bachelor's degree in a related field (Social Work, Nursing, Healthcare Administration) or equivalent experience.
  • Previous experience in care coordination or a related role in the healthcare industry.
  • Strong organizational, communication, and interpersonal skills.
  • Knowledge of home care regulations and industry best practices.
  • Ability to handle and resolve conflicts effectively.
  • CPR and First Aid certification preferred.
  • Passion for providing outstanding care to clients.

Preferred Skills:

Clinical Expertise:Proven experience in providing direct client care and ensuring safety.

Ability :Ability to develop /implement and evaluate organizational and client focused documentation in and system.

Government Funding:Good understanding of the government funding environment for aged care and NDIS (not compulsory but preferable).

Hospital Discharge:Good understanding of the hospital discharge environment.

Health Assessments:Good understanding of health assessments and developing care plans to meet clients' care needs.

Multitasking and Time Management:Ability to multitask and manage time effectively.

Communication:Excellent verbal and written communication skills.

Computer Skills:Intermediate to advanced computer skills, including familiarity with CRM systems.
