About the role


We are seeking a values-driven and proactive Case Manager to ensure that clients with multiple and complex needs within the North West region receive an appropriate service response to assist them to transition out of the homeless system.


The Case Management position covers the North West region of Melbourne and will be based across both the Coburg North and North Fitzroy North sites.


This role sits alongside the following program areas:

  • Youth Services work collaboratively with access point in Hume Merri-Bek for people who are homeless or at risk becoming homeless, delivering a continuum of support services including programs with an increasing focus on intervening early to prevent stress in housing and homelessness.-
  • Tranmere St Youth refuge Fitzroy North
  • supporting young people experiencing significant crisis and homelessness in supportive & therapeutic environments.
You will successfully

  • Provide a flexible and assertive case management response to clients with multiple and complex needs
  • Work collaboratively within Youth Services Integrated Model, having access to all programs and providing case management support
  • Conduct comprehensive assessments of a client's needs and circumstances both at the point of intake and ongoing through the support period
  • Provide intensive case management support to a caseload of young people who are experiencing high and complex needs
  • Work alongside the youth services team at the Sunshine youth hub to provide an assertive response to young people presenting in crisis
  • Participate in the YAC providing secondary consult support to the services present
  • Facilitate monthly seminars aimed at developing the capacity of staff within the Northwest region
This is a fixed term 12-month, full-time position. The operational hours are 9am-5pm and YS provides assistance to approximately 450 young people and families per annum.

You will have

  • Degree qualification in social work or related field. Relevant experience in a social service environment is preferred
  • Knowledge of youth homelessness, crisis intervention, and the ability to collaborate with other services within the North West region
  • Demonstrated skills in engaging & working with young people with complex support needs, specialist skills in youth AOD, mental health, out of home care & adolescent violence
  • Assessment skills & knowledge of a range of theories including bonding & attachment, trauma, child and adolescent development & strengthsbased approaches
  • A current VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
