Job location
Our Corporate Office is located at Thomas Embling Hospital just 15 minutes north of the city.

Due to our announced expansion across all Directorates, we have a need for experienced like-minded professionals to join our team to support our growing services.

Our Directorates includes Thomas Embling Hospital, Prison Operations and our Community Directorate with highly skilled disciplines including Nursing, Lived Experience, Occupational Therapy, Psychiatry, Psychologists and Social Work.

We have an outstanding L&D and discipline Educator co-hort that continually upskill our already skilled and supportive team.

Thomas Embling Hospital is just 15 minutes north of the city and working across all our directorates.

With significant growth planned for the team, our Lived Experience team are an integral part of implementing recovery approaches for mental health consumers and carers.

Lived experience workers bring expertise developed from their own mental health experiences.


About the Role:


This position is responsible for coordinating the organisation wide incident management system to support the identification, management and investigations of clinical risk and proactively supporting associated quality improvement activities.


The position will coordinate the high-level management of incidents (including sentinel events and serious adverse patient safety events (SAPSEs) and associated Statutory Duty of Candour responsibilities), monitor incident trends across the organisation, support the Complex Case review panel processes, monitor the implementation and evaluation of incident review recommendations, oversee the clinical audit schedule and provide support, coaching, education and guidance for staff in incident review processes.


Duties:


  • Provide high level leadership and consultancy services in relation to consumer safety and clinical risk management based on the effective review of clinical incidents and adverse events.
  • Oversee the facilitation of serious incident investigations to develop recommendations and strategies to prevent recurrence. This includes adherence to legislative obligations, team commissioning, investigation, and support of review panel staff to ensure delivery of high quality, detailed and accurate reports.
  • Actively participate in indepth reviews as required.
  • Provide mentoring, education, guidance, and ongoing coaching in the review of serious incidents and follow up actions to build staff capacity.

About You:


  • Demonstrated significant experience in Clinical Governance, accreditation, safety, quality and risk management systems
  • Significant, demonstrated experience in serious adverse event review methodology, including leading and facilitating review processes
  • Excellent interpersonal, communication and negotiation skills, with the ability to work collaboratively with multidisciplinary staff, senior clinicians and executive
  • Experience with auditing processes

What you need:


  • Tertiary degree qualification (or equivalent) in a relevant health discipline and registered with AHPRA (Australian Health Practitioners Regulation Agency) (nursing, psychology, occupational therapy) or eligible for registration with the AASW (Australian Association of Social Workers) (social work)
  • Experience with the incident reporting system VHIMS (Victorian Health Incident Management System) _Riskman_
