Community Connecting Illawarra is a community-based, person-centred support service for people with disabilities in the Illawarra region. We are an innovative organisation that embraces community inclusion for our members in mainstream environments.
We are currently offering an opportunity for a Support Coordinator.

We are currently looking to hire a Support Coordinator who is passionate about helping empower people with disabilities to live a full and meaningful life of their choosing.


The Role:


The role of a Support Coordinator is to connect participants with an NDIS plan to supports and services in the community.


  • Help our members understand their NDIS plan
  • Manage resources effectively to get the best outcome for our members
  • Work with other people to meet goals and outcomes (family, friends, service providers, therapists and community and mainstream supports)
  • Problem solve issues when they arrise
  • Monitor progress, outcomes and quality of services and supports
  • Provide reports to the NDIS
  • Assisting with NDIS review meeting.
  • Strong commitment to upholding the rights of people with a disability;
  • Demonstrated experience in using a personcentred approach to building trust and positive relationships with individuals, their families and extended support network;
  • Strong verbal and written communication skills;
  • Demonstrated leadership, problem solving and conflict management skills.
  • Flexibility to work in different working environments and undertake evening and weekend work when required;
  • Genuine enjoyment in the work that they do and authenticity in their service delivery;
  • Confidence in working with people with intellectual disabilities and other disabilities;
  • Ability to work independently
  • Knowledge and understanding of the NDIS.

Essential Criteria:


  • Valid NSW Driver's License and access to a vehicle;
  • Satisfactory NDIS worker screening check and Working with Children's check;
  • Bachelor's degree in social science, human sciences or equivalent;
  • Minimum 2 year relevant work experience

Salary:
$73, $77,577.00 per year

  • Monday to Friday
  • No weekends

  • Wollongong, NSW 2500: Reliably commute or planning to relocate before starting work (required)

  • Working with Children Check (required)
