Community Care Manager (Non-Clinical) - Brisbane
We are currently looking for a Community Care Manager to work for a well-established community provider located in Brisbane.

• Full time role
• Monday to Friday 8am4pm
• Overseeing a Home Care Package Portfolio (no CHSP)
• Covering Community clients in and around South/East Brisbane
• The role is supported by Client Coordinators who assist with all the admin work
• Manage your own workload mix of being in the office and out and about in the community
• 2 Head Office locations to chose from South Brisbane or East Brisbane

- $80-$85k + super + car

• Private vehicle for both work and private use

Overview of the role_

• Meeting and communicating with potential clients and their families to get an understanding of their current needs.
• Creating care plans for the community clients to ensure the highest level of care is being provided.
• Building relationships with other health providers and promoting the services that this organisation provides in the local community.

What we are looking for:_

• A Diploma of Community Services or equivalent
• Looking for Non-Registered Nurses as the role is not clinical
• Must have a good knowledge of the Australian Aged Care Standards
• Previous experience with Home Care Packages
• An organised individual who can manage their own work load
• A good communicator who can build relationships clients and families
• Someone who is passionate about Aged Care and wants to help improve lives to keep the clients living comfortably in their own home
• Have a valid driver's license and the right to work in Australia
