The Opportunity


We are looking for a
Care Manager to join our supportive and professional multidisciplinary teams in a
Full Time capacity on a
12 month contract.



Responsibilities include but are not limited to:
Providing quality case management services by embracing contemporary practices and compliance with Aged Care Quality standards.

This service will be delivered in a manner that:

  • Is in accordance with the values, policies and procedures of HealthAbility
  • Follows a consumer directed care and wellness and reablement practices
  • Reflects current knowledge and best practice in aged and community services
  • Fosters best practice and effective performance by staff and ongoing learning environment
  • Provides support and advice to home care package consumers and their carers to support them to achive their goals and remain living at home and in their community in accordance with the Home Care Package Operational Manual.
  • Developing and documenting consumer directed care plans and assessment with regard to their needs and preferences
  • Provide ongoing care management for Home Care Packages (HCP) for the consumer/carer, conduct regular reviews of their care needs and goals, amend care plans and supports and services as appropriate.
  • Liaise and advocate with other relevant service providers regarding individual care needs.

Skills & Experience

  • Minimum of 3 years' experience in Home Care Packages program and/or Tertiary qualification in Social Work, Allied Health, Nursing, Care Management or equivalent
  • Extensive experience working with older people and/or with Home Care Packages consumers
  • Demonstrated experience in Consumer Directed Care (CDC) choices and preferences to consumers
  • Knowledge of aged care related specialist services, i.e. Parkinson's and Dementia support Services would be an advantage
  • Knowledge of budget management and fees and costs associated
