As the Allied Health Assistant, you will participate as a member of the Virtual Rehabilitation Ward team, which provides care 7 days a week to people with complex medical, functional and/or psychosocial needs in their own home in lieu of an extended or avoidable hospital admission.

Your role will include the provision of multidisciplinary assessment, rehabilitation interventions, goal setting and care planning in collaboration with the patient, their family, carer, general practice and existing community services involved in their care.

You will operate under general direction and with limited supervision by the professional Allied Health (AH) staff in the interdisciplinary team.


To be successful in this role, you will have previous experience working in health care, preferably with in an acute or sub-acute rehabilitation setting, experience working with a diverse range of people, the ability to communicate effectively with all levels of staff, patients and external agencies both orally and in writing, and Certificate 4 in Allied Health Assistance, or an equivalent qualification.
