The Position

Position Title:
Lead Clinician, Older Persons Community Mental Health Team


Classification:
Registered Nurse Grade 3, Occupational therapist Grade 2 & Social worker Grade 2


Salary per hour (exc. super): dependant on qualifications


Hours per fortnight: 80 (including ADO every 4 weeks)


Employment Status:
Full Time, Fixed term

Role Description


The Older Persons Community Mental Health Team provide community based mental health assessments, treatment and support to patients who are aged over 65 years suffering from mental illness or aged over 65 and suffer from a mental illness complicated by a neurodegenerative disorder and/or issues of ageing and/or severe comorbid physical illness.


The successful applicant will provide mental, social and physical assessment, care coordination, clinical treatment and support for consumers, their families and carers.

In conjunction with the manager and senior clinicians, the successful applicant will support and/or provide a range of bio-psychosocial treatment modalities including individual, family and group settings in the community.


An exciting opportunity to identify and co-ordinate key community agencies and service providers involved in the provision of recovery, support and discharge planning consistent with the patients and carers needs.
