The exciting opportunity:


  • Work across Perth's Northern suburbs
  • Responsible for overseeing and coordinating the onboarding process of new Home Care Package clients for the assigned region
  • Liaises with key internal stakeholders to determine the service capacity and the suitability of potential Home Care Package clients
  • Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options
  • Ensures that information provided to clients pertaining to Consumer Directed Care (CDC), package levels, fee structures and service agreements, are relevant and accurate
  • Maintains appropriate documentation, ensuring that it reflects professional standards
  • Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements

About you:


  • Must have previously held, or have current registration with AHPRA as a Registered Nurse or Allied Health Professional (Occupational Therapist, Physiotherapist, etc.)
  • Have experience in community aged care and aware of the Home Care Packages Program and Commonwealth Home Support Program including the funding elements
  • Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
  • Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
  • An energetic and enthusiastic leader who inspires and supports their team
