The exciting opportunity:

Work across Perth's Northern suburbs
Responsible for overseeing and coordinating the onboarding process of new Home Care Package clients for the assigned region
Liaises with key internal stakeholders to determine the service capacity and the suitability of potential Home Care Package clients
Develop and maintain mutually beneficial relationships with external stakeholders so clients have access to numerous social opportunities and lifestyle options
Ensures that information provided to clients pertaining to Consumer Directed Care (CDC), package levels, fee structures and service agreements, are relevant and accurate
Maintains appropriate documentation, ensuring that it reflects professional standards
Monitor and manage client budgets ensuring they comply with their approved package levels and individual requirements

About you:

Must have previously held, or have current registration with AHPRA as a Registered Nurse or Allied Health Professional (Occupational Therapist, Physiotherapist, etc.)
Have experience in community aged care and aware of the Home Care Packages Program and Commonwealth Home Support Program including the funding elements
Fantastic customer service and negotiation skills with a proven track record building and maintaining relationships with clients
Committed to using a client directed model of care to empower clients and encourage reablement and community engagement
An energetic and enthusiastic leader who inspires and supports their team
