TaylorCare Recruitment is proud to partner with a Not for Profit to recruit an experienced team leader within Disability and Out of Home Care where you will join the team as Disability Service Manager where you will make an Impact in the lives of participants in SIL & SDA homes then this will be the ideal role for you based in Macquarie.


Competitive salary package:


Salary range $85k - $95K + super + salary packaging.


Work for a Not-for-Profit Organisation who provide that services people with physical and psychosocial disability since the 1950s and they are also a registered NDIS provider offering integrated clinical, health, social and disability services in accommodation, community participation, supported employment and support coordination services in the Greater Sydney and in Northern Rivers regions in NSW.


As the Disability Service Coordinator where you will Coordinating and overseeing the delivery of clinical services to our participants, ensuring their needs and goals are met effectively.

Health related goals will be a primary focus of this role.

You will collaborate with participants, their families, support teams and allied health professionals to develop individualized care plans that align with NDIS goals and guidelines.

You will conducting comprehensive assessments of participants' clinical needs and developing appropriate service plans accordingly.

Collaborating with other stakeholders, such as NDIS coordinators, allied health professionals, and community organizations, to enhance participant outcomes and promote a multidisciplinary approach to care.

To be eligible for the

  • A Bachelor's degree in a relevant field such as Nursing, Social work, Psychology, occupational therapy, or a related discipline is often required.
  • Demonstrated experience and knowledge in providing clinical services, preferably in the context of disability support and the NDIS, SIL & SDA models.
  • Experience with complex clients and restrictive practices, therapeutic interventions is essential.
  • Flexible work place being able to drive between sites and offsite meetings where required
  • Internal and external stakeholder engagement and networking to ensure seamless integration into the service.
  • Conducting Assessments of participants clinical needs and support plans
  • Providing clinical guidance, supervision & training to support staff to maintain a high standard of care
  • Experience managing and supervising staff and client outcomes in multiple accommodation services
  • WWCC, National Police Check, NDIS Screening, Drivers Licence
