The Role

The role of this position is to work collaboratively in multidisciplinary teams around Melbourne to provide clinical support to clients referred to us by our network of General Practice Clinics.

They will provide education and advice in diabetes self-management to a range of clients in clinic settings.

We have locations in Croydon, Essendon, Mulgrave, Newport, Kingsville, Tarneit, Mill Park, Point Cook, Werribee, Sunshine, Williamstown, Glen Waverley, Warragul, Keysborough, Dandenong South, Dandenong North, Emerald.

Roles and responsibilities not limited to:

• Provide high quality, evidence based, diabetes self-management education, advice and support to clients.
• Provide insulin initiation and titration support as per instructions from the client's GP/Specialist.
• Assist and educate patients in the use of relevant blood glucose monitoring.
• Liaise with relevant agencies and organizations concerning planning and delivery of diabetes management modules/courses equipment.
• Provide written reports to clients' general practitioners following the initial assessment, progress reports and discharge summary.

Essential Requirements

• Relevant tertiary health professional qualification (RN, Division one, Midwife, Pharmacist, Podiatrist, Physiotherapist, Practicing Dietitian, Exercise Physiologist) and current registration with the appropriate governing body.

Credentialed Diabetes Educator status and
current registration with the ADEA.

• Proven ability to work independently and as part of a multidisciplinary team.
• Eligibility for a Medicare provider number.

What We Offer

Flexible Part-Time work with close to fully booked appointments of more than 3 months, simply drop in and start working full days right away.

Attractive salary of $40 an hour with incentive bonus of $5 for every patient attended to when more than 15 patients a day are seen.
