Tweed Community & Allied Health are pursuing a Health Clinician to Provide case management, coordination of care, identification of required team members and discharge planning for clients on the Transitional Aged Care Program.

Providing advanced clinical services, leadership and support as an integral part of the multidisciplinary health care team for patients/clients of the Northern NSW Local Health District (NNSWLHD).


This vacancy is maternity leave relief for an initial period of up to 57 weeks for 38 hours per week; the hours and period of employment may change according to the maternity leave requirements.

There is also the possibility of an extension at same / reduced hours.


_If you have/are:
_


  • Occupational Therapist / Physiotherapist / Social Worker / Dietitian / Speech Pathologist relevant tertiary qualifications and registration with the Australian Health Practitioner Regulation Agency (AHPRA) or eligible for membership with AASW / DAA / SPA as appropriate; OR Registered Nurse (Division 1) with the Nursing and Midwifery Board of Australia with a postgraduate qualification in Aged Care; and at least 3 years experience in a clinically relevant area
  • High levels of experience providing comprehensive needs assessment, care and service planning and evaluation for patients with complex needs in partnership with clients, their carers and service providers
  • Evidence of clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care
  • Commitment to excellence in practice relevant to qualifications, professional development and the provision of education and supervision to others
