This varied role will see you supporting Older Adult Clients with a view of maintaining their independent living according to their preferences.


  • Team Support_

Reporting to the HCP Team Leader you will:

  • Assist in the coordination of the service
  • including clinical systems, processes, clinical risk, team engagement.
  • Collaborate with the Manager NSW Older Adult Services and Older Adult team members on the development and implementation of the program strategy and budget.
  • Client Support_

Clinical Case Managers at EACH are expected to work collaboratively with the customer and their support network to monitor and review the individual plan, ensuring it remains responsive to changing needs and circumstances and within an agreed budget plan.

, you'll focus on creating opportunities for equitable outcomes for our clients by:


  • Providing Home Care Package to older adults with enhanced access to holistic supports.
  • Connecting them with professionals to assist them with occupational therapy needs, home equipment requirements, domestic assistance or community connections depending on their needs and their plan.
  • Building and maintaining collaborative relationships with referring doctors and specialists.
  • Taking part in regular clinical case review meetings and multidisciplinary team meetings to ensure appropriate clinical service pathways and service delivery.

Qualifications:

If you are an experienced Case Manager, qualified as a Nurse DIV 1, Occupational Therapist, or equivalent, confident working across multiple IT systems, and capable of effectively engaging individuals and families to build positive and sustainable relationships, then you might be the perfect person for our team

  • Experience and skills require:_
  • Highly developed Assessment and Care Planning skills with a focus on wellbeing and reablement.
  • Demonstrated experience effectively managing a brokerage budget, including invoicing, receipting and financial processing as required, and negotiating the budget with the clients.
  • High level assessment (initial and ongoing) and clinical care planning experience.
  • Mandatory Qualification/s, Competencies and/or Licences_
  • Current Registered Nurse DIV 1 with APHRA or equivalent.
  • Current statebased driver's license.
