About the Role
Monash Health is the largest integrated health service in Melbourne and has world renowned research and teaching facilities.
The Young Adults Transition service delivers comprehensive care to adolescents with complex disability who are transitioning from paediatric to adult services.
This position is required to lead this multidisciplinary team both operationally and strategically, in collaboration with the manager and other clinic leads within the Sub Acute and Ambulatory Care, Specialist Clinics.
Key days are Tuesday and Wednesday.- Leadership role

What you need:
To be considered for this position you need (Italics include desirable):
- Degree in relevant Allied Health Profession (PT/OT/SW)
- Multidisciplinary team experience
- Awareness of factors affecting disability and transition in the health setting
- Exceptional communication skills
