In this full-time position reporting directly to the Clinical Care Manager, you will be at the forefront of our home care operation, wielding your expertise and dedication to ensure its unparalleled success.

The Care Coordinator will be responsible for:

  • Building strong connections with clients and their families
  • Assigning staff to clients based on staff qualifications and availability, as well as client preferences
  • Organise support to clients with disabilities and monitoring and reporting on their progress
  • Referring clients to agencies that can provide additional help
  • Assessing client needs and resources for health, welfare and other facilities and services
  • Maintaining accurate records of incidents and feedback as required
  • Reorganising work priorities to meet the needs of a fastpaced environment
  • Supporting all clients to meet their goals
  • Conducting Client Care Assessments and Care Planning
  • Providing clinical guidance to other team members for client management
  • Discussing and promoting Right at Homes services with medical professions and referral sources
  • Ability to work in a fastpaced, dynamic environment

Qualifications:


  • Qualified Nurse
  • Commitment to a culture of teamwork, compassion, and service excellence
  • Strategic thinking abilities
  • Understanding of the dynamics of a commercial operation
  • Demonstrated leadership skills
  • Strong judgment, quick decisionmaking, and effective staff management
  • Excellent communication skills
  • Some understanding of education and accreditation standards, Continuous Improvement, Occupational Health and Safety (OHS), and risk management
  • Current National Police Clearance
  • AHPRA Registration

Preferred Skills:


  • Communication skills (including patient compassionate listener)
  • Computer skills (Office Suite, SharePoint, Teams)
  • Coordination under pressure
  • Interpretation and sound judgement
