Your Role
As an Assistant Co-ordinator - Community Care you'll be looking after the most important people in our organisation - our clients.
Your tasks will include:
- Assess, plan, coordinate and review care delivery
- Enable clients to maintain as much independence as possible, and encourage them to use their abilities and initiative to develop existing and new skills
- Report and document changes in behaviour patterns, unusual actions or reactions of clients or employees
- Provide direct client care when required
- Assist with scheduling of employees to suit the needs of the client
- Liaise and network with other service providers and the community as directed

Role Requirements
- Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
- Current Police Screening Certificate
- Current First Aid and CPR Certificate
- Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
- Current Queensland driver's licence
- Tertiary Qualifications (as appropriate) and/or relevant experience in a healthrelated field (e.g. Health and Human services)
- Experience in case management and client care coordination
- Demonstrated sound verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
- Flexibility to work across a range of days (Monday to Sunday)
