Your Role

As an Assistant Co-ordinator - Community Care you'll be looking after the most important people in our organisation - our clients.

Your tasks will include:

Assess, plan, coordinate and review care delivery
Enable clients to maintain as much independence as possible, and encourage them to use their abilities and initiative to develop existing and new skills
Report and document changes in behaviour patterns, unusual actions or reactions of clients or employees
Provide direct client care when required
Assist with scheduling of employees to suit the needs of the client
Liaise and network with other service providers and the community as directed

Role Requirements

Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
Current Police Screening Certificate
Current First Aid and CPR Certificate
Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
Current Queensland driver's licence
Tertiary Qualifications (as appropriate) and/or relevant experience in a healthrelated field (e.g. Health and Human services)
Experience in case management and client care coordination
Demonstrated sound verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
Flexibility to work across a range of days (Monday to Sunday)
