The primary objective of this role is to plan, co-ordinate and provide day to day administration of Commonwealth Home Support Programme (CHSP) in-home/individual services for consumers and ensure the activities are operated effectively and efficiently to meet the identified needs of frail elderly people & their carers.

Total hours per fortnight: 30 hours / fortnight

The Requirements:
_ Essential:
_
- Minimum 3 years working experience in aged or community based services for older people
- Tertiary qualifications in aged, disability or related area e.g. Nursing, Community, Health, Welfare or attained Certificate IV in Care Support Services / Aged Care, case management
- Demonstrated ability to work independently and work well with others
- Excellent interpersonal and time management skills
- Speak fluent English/Cantonese/Mandarin and good command of English and with good report writing skills
- First Aid Certificate or willingness to obtain one
- Current full driver license
- National Police Clearance
- Australian Citizenship / Permanent Residency.

- Experience in Home Care Services /dementia care /respite care
- Experience in case management
- Experience in leading a team
- Experience in working in a community based setting
- Experience in working with Culturally and Linguistically Diverse populations
