Client Advisor (Case Manager) you will be based out of our
-
Bendigo Office reporting to the Operations Team Leader. Your focus will be to develop a positive client experience in collaboration with the Rural North West Community Health and Care Team.


Your Key responsibilities will involve;

  • Assist clients in understanding the requirements of their package and the choices available to them.
  • Coordinate all services and supports in line with client requirements and needs.
  • Undertake client intake and assessments, including care planning, monitoring and regular reviews.
  • Provide a strength based and person centred approach to ensure client's independence is maintained.
  • Guarantee all services delivered are consistent with the funding, guidelines and contracted requirements.
  • Comprehensive documentation; case notes, care plans, correspondence and referrals
  • Forward thinking and planning to maintain and prioritise adequate allocation of resources.
  • Implement best practice quality models of assessment, care plan development, implementation and review.
  • Actively promote and refer clients to Benetas services

To thrive in this role you will have:

  • Excellent customer service and interpersonal skills, with the ability to demonstrate empathy and understanding to older clients and their families
  • A positive approach to working within a team and autonomously
  • Demonstrable experience in case management within aged care, home care, or allied health services (desirable)
  • Tertiary qualifications in a health or social servicesrelated discipline or related experience (desirable)
