Benetas has an exciting opportunity for an individual whom is highly organised, has proficient communication skills and who will support clients in navigating their Home Care Package. As a

Client Advisor (Case Manager) you will be based out of our
-
Bendigo Office reporting to the Operations Team Leader. Your focus will be to develop a positive client experience in collaboration with the Rural North West Community Health and Care Team.

Your Key responsibilities will involve;

Assist clients in understanding the requirements of their package and the choices available to them.
Coordinate all services and supports in line with client requirements and needs.
Undertake client intake and assessments, including care planning, monitoring and regular reviews.
Provide a strength based and person centred approach to ensure client's independence is maintained.
Guarantee all services delivered are consistent with the funding, guidelines and contracted requirements.
Comprehensive documentation; case notes, care plans, correspondence and referrals
Forward thinking and planning to maintain and prioritise adequate allocation of resources.
Implement best practice quality models of assessment, care plan development, implementation and review.
Actively promote and refer clients to Benetas services

To thrive in this role you will have:

Excellent customer service and interpersonal skills, with the ability to demonstrate empathy and understanding to older clients and their families
A positive approach to working within a team and autonomously
Demonstrable experience in case management within aged care, home care, or allied health services (desirable)
Tertiary qualifications in a health or social servicesrelated discipline or related experience (desirable)
