Me & My Support is an expanding contemporary Disability Support Service that operates as a Registered NDIS Provider within the Brisbane, Gold Coast and Sunshine Region.

Me & My Support aims to deliver person-centred support that promotes the safeguarding principles of safety and quality when supporting people to live an enriched life.

Me & My Support has a diverse range tailored support options to cater for people goals which includes, but not limited to Supported Accommodation options and other lifestyle services funded by the National Disability Insurance Scheme (NDIS).


About the position


The Service Delivery Coordinator will play an important role with coordinating the organisations day to day operations across the organisation.

The Service Delivery Coordinator will report to, and work closely with the Service Delivery Manager to ensure smooth delivery of services across the organisation.


The Service Delivery Coordinator will represent and contribute to managing and coordinating effective relationships with Stakeholders, the Community, Participants and their Families or Nominees.


Key Tasks & Responsibilities

People Management

  • Under the supervision and guidance of Service Delivery Manager, provide mentoring and hands on leadership to operational Staff, inclusive of Disability Support Workers and Team Leaders
  • Engage in rotational on call duties as directed
  • Support the Service Delivery Manager with workforce strategic planning, developing and implementation, in consultation with other leadership members

Service Delivery

  • Manage and coordinate services that are being delivered by the organisation, including SIL Homes and other Lifestyle Services as required
  • Ensuring that Participants Support Plans, Risks Assessments, and other supporting service delivery documentation is up to date, and are compliant with industry practice standards
  • Support the Service Delivery Manager with actioning Incident Management tasks, including reporting requirements to the NDIS Quality Safeguard Commission via the NDIS Portal
  • Create and manage Participant Service rosters that reflect Participant Support needs, by assigning suitable Disability Support Workers
  • Liaise and action requests from Support Coordinators regarding Participant matters as required
  • Engage and contribute to management meetings and other continuous improvement activities
  • Contribute to creating strategic operational plans in consultation with other leadership members.

Community Engagement

  • Engage and represent the organisation in networking meetings, expos and other opportunities that enhance the reputation and exposure of Me & My Support.
  • Provide networking written and verbal updates to Service Delivery Manager

Position Criteria

Experience, Attributes & Skills

  • Experience in a similar role, highly desirable
  • Leadership experience (highly regarded)
  • Experience coordinating supports for people with a disability under the NDIS
  • Managing SIL Homes
  • Rostering and scheduling experience
  • Positive and contemporary attitude
  • Ability to maintain professional development
  • Good knowledge of NDIS Price Guide & NDIS Practice Standards
  • Good knowledge of the Community & Home Care Award (SCHADS Award)
  • Experience with Incident & Conflict Management
  • Sound written and verbal communication skills
  • Ability to work and contribute within a diverse team setting
  • Ability to work independently within a fastpaced environment
  • Good I.T Skills, confidence to navigate work Apps, SharePoint & Adobe Sign

Qualification & Licenses

  • NDIS Worker Orientation Certificate
  • Blue Card
  • NDIS Worker Clearance
  • Diploma level in Community Services or similar (Highly Desirable)

Salary:
$70, $80,000.00 per year

  • 8 hour shift
