Description

We are seeking a highly motivated and dedicated individual to join our team as a Chinese-Speaking Support Coordinator in the NDIS (National Disability Insurance Scheme) sector.

As a Support Coordinator, you will play a crucial role in assisting individuals with disabilities and their families to navigate the NDIS system and access the necessary supports and services to enhance their quality of life.

Responsibilities:

Coordinate Support Services:

Work closely with NDIS participants, their families, and relevant stakeholders to identify individual goals, develop support plans, and coordinate the provision of services.

Ensure that participants receive appropriate supports based on their needs and preferences.

Culturally Responsive Communication:

Provide support and guidance in Mandarin or Cantonese to Chinese-speaking participants, effectively explaining NDIS processes, services, and available supports.

Foster trust and rapport with participants and their families by delivering culturally sensitive and personalized assistance.

NDIS Navigation:
Assist participants in understanding their NDIS plans, funding budgets, and available service providers. Support participants in making informed choices and decisions regarding their supports, ensuring their goals are aligned with their plan.

Service Provider Engagement:
Collaborate with a range of service providers, including disability support organizations, health professionals, and community resources. Facilitate participant referrals, advocate for participant needs, and monitor the quality of services being delivered.

Documentation and Reporting:

Maintain accurate and up-to-date records of participant interactions, service plans, progress reports, and relevant documentation in compliance with NDIS guidelines.

Prepare reports and contribute to ongoing evaluation and quality improvement initiatives.

Requirements:

Fluency in Mandarin or Cantonese:
Proficiency in both written and spoken Chinese languages is essential to effectively communicate with Chinese-speaking participants.

NDIS Knowledge:
Sound understanding of the NDIS framework, including eligibility criteria, funding options, and support categories. Experience working in the NDIS sector is highly desirable.

Support Coordination Experience:
Proven experience in providing support coordination or case management services, particularly within the disability or community services sector.

Cultural Competence:
Demonstrated ability to work with diverse cultural communities, understanding and respecting cultural values, practices, and beliefs.

Excellent Communication Skills:

Strong interpersonal and communication skills, with the ability to build rapport, actively listen, and convey complex information in a clear and understandable manner.

Problem-Solving and Organizational Skills:

Effective problem-solving abilities, organizational skills, and attention to detail to manage multiple participant cases, prioritize tasks, and meet deadlines.

Qualifications:
A relevant qualification in disability services, social work, psychology, or a related field is highly desirable.

NDIS Worker Screening Check:
Successful completion of the NDIS Worker Screening Check or willingness to undergo the process.
