What You Will Be Doing


The Peninsula RAS Home Support Assessor is responsible for conducting holistic and timely assessments of older clients considering their physical, medical, psychological, cultural, social, environmental and wellness dimensions of care.

The Peninsula RAS Home Support Assessor works closely with the client to assess their goals, needs and preferences with a focus on their functional and Re-ablement potential to allow them to maintain their independence and remain in their home for as long as possible.


The Peninsula RAS Home Support Assessor will work as an independent practitioner within the multi-disciplinary Peninsula Regional Assessment Service (RAS) to provide Home Support assessments for aged members of the community and their carers.

The ACAS and RAS are delivered as a part of My Aged Care (MAC).


What You Need

We anticipate you will have:

  • Diploma in Nursing OR Diploma in Community Services
  • Registered with AHPRA as appropriate
  • Current Victorian Driver's License and ability to deliver service across catchment
  • Demonstrated knowledge of the aged care client and issues related to ageing
  • Ability to conduct holistic assessments within a Wellness and Reablement framework
  • Demonstrated high level proficiency in the use of IT systems including Microsoft Office (Outlook, Excel, Word/typing skills) and electronic client management systems
  • Excellent communication skills (using thorough written and verbal modes) as well as a professional telephone manner
  • Sound organisational skills
  • Ability to work onsite and from home
  • Demonstrated ability to work autonomously as an effective member of a multidisciplinary team
