CCA NSW is a busy, non-profit community organization, providing services to frail aged/ disability clients for over 55 years.

We are seeking a vibrant, collaborative, detail-oriented worker to support elderly people and people with disability to continue living in their own homes while maintaining their independence and connection to the community.

This is an exciting opportunity for someone looking to learn and grow in both the NDIS and Aged Care Sectors.

About the role:

This is a Full-Time position based in our Eastwood office.

Your new role, as an Aged and Disability Services Client Care Coordinator, entails supporting both NDIS and aged care clients to access and engage services that build their capacity and maintain independence and individuality.

Qualifications & Experience

• A Tertiary qualification in relevant field, or extensive, relevant experience
• Minimum 1 year Support Coordination or Local Area Coordination experience
• Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
• Drivers license and access to a car.
• Bilingual skills will be a benefit

Tasks & Responsibilities

• To effectively liaise and support the elderly and those with a disability and their carers
• To effectively coordinate CCA's NDIS program and ensure CCA maintains its NDIS registration.
• To provide Support Coordination to NDIS participants
• To deliver client care through assessment of clients, assist walk in clients and track expected outcomes of clients.
• As this role is in the ageing and disability space COVID vaccination is required.
