About the Role
_ This is a full time (35 hours per week), closed period contract until 2025. This role will be based in Campbelltown but will work across the broader South West Sydney region. _
This is an exciting opportunity to be part of a new initiative in partnership with Karitane and South West Sydney Local Health District developed to support young parents and their children.
In this management role you will be responsible for the oversight and delivery of the Key Worker team as part of new SWS pilot program.
This initiative is targeted towards vulnerable young parents by identifying services through a stepped model of care to address their individual needs by providing information, advice assessment case planning, service coordination and engagement.
Key responsibilities include:
- Provide efficient and effective coordination of the SWS Key Worker model
- Develop and ensure in collaboration with the program leadership team a rigorous practice framework, process, procedures and strategies for working with family and children within the Key Worker model
- In partnership with Sonder and Karitane, develop and implement referral and intake pathways and processes that identify and support young parents who are most likely in need of the key worker model
- Establish strong networks and partnerships within the South West Sydney service system
About You
- Previous management experience within the community services sector with at least 2 years managing a small team of specialists
- Demonstrated experience in case work, case management, and coordination in a child and family context
- Strong understanding of current child protection legislation and mandatory reporting
- Experiencing in developing and implementing systems, systems, processes and practice frameworks
- Capacity to develop and maintain strategic partnerships to deliver the key worker model
- Extensive knowledge of the service system with SWS and existing stakeholder relationships (Desirable)
- Experience in delivering services to vulnerable people
