Our home care services include companion, personal, nursing, respite, transportation, aged, disability, transitional, 24 hour, and palliative care.

Purpose of Position:

This role is customer focused and involves consulting with families, preparing in home care assessments, and drafting care plans to best meet client requirements.


Duties include:

  • Dealing with a range of seniors and frail elderly customers and their families on a daily basis. This role will handle anything from information requests, feedback, general inquiries to complaints.
  • Ensuring all client contact is managed in an empathetic, friendly, interested and patient manner to quickly build rapport, trust and confidence in Right at Home.
  • Providing client and family education on the nature of services that are available and the effect and benefit of the care plan.
  • Preparing inhome care assessments and drafting care plans to best meet client requirements.
  • Managing complex government funded cases as well as private pay clients.
  • Recruiting, training, and managing the performance of the care team.
  • Participating in community activities or committees as requested.
  • Ensuring highest quality service is consistently delivered.
  • Assisting management with business development opportunities across a range of referral sources.
  • Operate and maintain care planning and scheduling technology.
  • Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks.

Qualifications:


  • National police check
  • Current First Aid and CPR
  • Reliable source of transportation required
  • Comprehensive Car Insurance
  • Certificate 3 or higher in Aged or disablility care
  • If EN or RN
  • Current AHPRA registration & Professional Indemnity Insurance

$55, $75,000.00 per year
