About the Role

Chronic Disease services operate across Brisbane South, Bayside and Logan areas, with service delivery provided in the most appropriate setting for the patient, this could be the patient's domestic residence, community health setting, outpatient setting or as an acute inpatient.

The principle objective of the Chronic Disease service is to reduce risk of long-term complications, facilitate early discharge from hospital and reduce the risk of hospitalisation for patients with complex chronic disease conditions.

This position will contribute to patient care by providing administrative and clinical assistance to allied health professionals (AHP) in their day-to-day clinical duties.

Works under the supervision and direction of the AHP and undertakes only those clinical tasks delegated to them.
