Description

The National Disability Insurance Scheme (NDIS) supports people living with a broad range of physical, intellectual, and psychosocial impairments by providing packages of assistance.

These packages outline a plan and budget that is able to be used, to access a range of supports a person needs.

At times, understanding how this budget is utilised and allocated can be tricky, and this is where a Support Coordinator comes in

A Support Coordinator works directly with people receiving NDIS funding and their families, to remove any barriers in navigating their package and establishing supports.

This assistance aims to build capacity and confidence around coordinating and managing their own supports, over time.

A key ally in this person-centric model of service, you will provide support by:

  • Ensuring funding is being utilised and allocated as per a person's NDIS plan.
  • Developing networks of support service providers in the community, and
empowering people to exercise their choice and control in their provider.
  • Advocating for individuals and their families with
dignity, communicating their goals and aspirations to their stakeholder team, and the National Disability Insurance Agency.
  • Attending home and community visits to connect, build relationships and capacity, and ensure that the quality and value of the supports received, meets expectations.
  • Ensuring all the necessary "paperwork" including case file notes and reporting is completed in a timely manner, to meet NDIS and sector requirements and guidelines.
The skills you need to be successful in this role include:

  • An adaptable and flexible communication style that stems from a place of support, service, and
    integrity:
  • Able to connect and build meaningful relationships, empathise with,
    and
    empower others
  • An ability to manage and pivot comfortably between a variety of competing tasks
  • Organisation; being meticulous with "paperwork" on a variety of online platforms. This includes reporting, recording notes, and other internal and external processes for follow up.
  • Collaborating and problemsolving with internal and external stakeholders

If this role interests you, we currently have an opportunity for a full-time (Monday - Friday) permanent, Support Coordinator based out of
Nudgee, in North Brisbane.

As a guide, the salary on offer commences at a Level 4 under the SCHADS Award, which equates to $77, % Superannuation + Benefits (as outlined below) per annum.

Any experience working in a similar role in the Care Sector or in Human Services where you have had experience in case management is ideal.

However you must have a strong sense of
compassion toward others and are keen to learn, grow and make a difference
