Your Opportunity


Mission Australia's Drug and Alcohol Youth Service (DAYS) provide young people and their families' access to a comprehensive range of free and confidential alcohol and other drug services.


Residential Rehabilitation aims to provide young people with a friendly, supportive, and structured environment to complete a drug and alcohol rehabilitation program.

Its focus is to provide both physical and emotional safety while they engage in treatment to transition back into the community.


  • It is the only youthspecific withdrawal unit in Western Australia._
The Aftercare Case Manager provides support to the young person after they have completed the residential program.

You will work to sustain progress made at DAYS and minimise relapse of Alcohol and Other Drugs, as well as engaging young people to address their physical, social, and emotional needs.


Your Key Responsibilities

  • Respond to referrals from internal support services and coordinate case allocations
  • Complete assessments and intake processes for allocated people, including all necessary documentation and data entry
  • Work with young people to build relationships centred in honesty and trust to create holistic and strengthbased individualised case/support plans utilising SMART goals and referral to supplementary services as needed
  • Assist people to transition out of the service into independence or other services including developing the capacity to selfmanage and access required supports independently
  • Provide advocacy and referral to appropriate services enabling young people to achieve their individual goals
  • Facilitate service information and harm minimisation education sessions with young people, families and agencies as required
  • Provide on call support to afterhours services
  • Update case notes and use appropriate record keeping ensuring compliance in the program
  • Collaborate with the team in the care and management of youth at risk

Requirements for Success
With a significant background in the _AOD and/or child, youth & family _sectors, you will be confident in your case management and assessment processes


Other requirements include:

  • A Degree or Diploma in Social Welfare/Social Science/Youth Work or a minimum of three years' experience in an AOD role.
  • Prior relevant experience and sound understanding of issues facing youth and AOD challenges
  • Welldeveloped communication, interpersonal skills, and the ability to tailor communication appropriate to the audience.
  • Ability to provide outreach engagement and support with young people experiencing risk factors in their life. This will require perseverance, flexibility, and innovation.
  • Highly competent administration and reportwriting ability.
  • Must have a Full driver licence with no restrictions.
  • Ability to provide evidence of up to date COVID19 vaccinations.


Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
