The Role
Our health centre at the

Ravenhall Correctional Centre is looking for a full-time Primary Care Coordinator with a

  • Discharge Coordination _focus to support our passionate and driven primary care team.


Reporting directly to the Health Service Manager the role has the autonomy to support a busy and diverse team as a clinical leader by providing effective oversight of the daily operations of the health centre, and uses clinical expertise to provide guidance, direction, and supervision.


You will have oversight of the daily operations of the medical centre and will demonstrate clinical expertise to provide guidance, direction and supervision, to the primary care team at Ravenhall, as well as supporting the Health Service Manager in overall effectiveness and management of the clinical environment.


This role will also oversee the Discharge Planning portfolio which includes ensuring coordination of discharge services and continuity of care for patients upon release from the correctional facility.


Essential experience and attributes include the following selection criteria:

  • Division 1 Registered Nurse qualification
  • Registration with the Australian Health Practitioner Regulation Agency
  • Substantial clinical experience including a minimum of three years post graduate experience
  • Experience with discharge planning and coordination
  • Recent nursing experience in an acute and/or primary healthcare setting
  • Demonstrated ability to clinically lead a successful team in a primary healthcare setting
  • Superior communication, interpersonal and organisational skills
  • Ability to assist and support the implementation of quality and change management initiatives and clinical projects
  • Intermediate to advanced PC skills
