About the Social Support Group
***As a Lifestyle Coordinator you will design amazing life experiences for our clients Social Support Groups, provide a group community based activities outside the home run by CBCHS. We provide respite and support for carers of people who are frail, aged or with a range of conditions including Dementia, Parkinson's, MS and acquired brain injuries.

  • We aim to develop program activities that ensure clients have a say in planning, are encouraged to maintain their independence, and activities that are stimulating, purposeful and fun. We want our clients to look forward to their scheduled day(s) and to go home feeling satisfied with their experience.

Current programs include:
outing based activities, exercise programs and centre based activities.

Lifestyle Coordinator
  • Community Health will work side by side with the Social Support Group (SSG) Team to ensure program planning and implementation is well coordinated, with the aim that clients receive well designed and executed social support activities. Strong capability to follow processes, support and mentor staff/volunteers and have had experience in designing, oversight, and implementation of a wide range of stimulating programs. Our clients are generally over 65yo, live at home and are wanting to connect with other likeminded individuals. We also support clients with cognitive changes and Cultural Groups within Kingston and Bayside.

0.8FTE or full time permanent basis. With flexible hours to suit you between 8.30am and 4.30pm, no late nights or weekend work required.

  • The SSG Team will be implementing new program initiatives in 2023, therefore we expect the Lifestyle Coordinator will be involved in developing new programs including community engagement, contribute to marketing campaigns, and review of new initiatives.
    Key Responsibilities
***- Work with Group Supervisors to ensure a consistent approach to the planning, coordination & implementation of SSG programs. This includes monitoring programs developed by Group Supervisors to embed the wellness and re-ablement principles into program plans with programs supporting independence, social connection, and promoting a sense of purpose and fun for group participants.

  • Design, preplan and implement programs for implementation by Group Supervisors and/or Support Workers as directed by Wellness & Social Support Coordinator. This includes developing program timetable, activity programs based on participant choice that are appropriate to their needs and aligned to funding guidelines. Program development may include online group participation options.
  • Assist Group Supervisors with preplanning of activities including café bookings, purchasing of supplies and venue hire.
  • Where no Group Supervisor is allocated to the program, monitor Group Programs, and assist with client transportation / group facilitation as required. This includes supervising support workers assisting with the program and ensuring understating of the planned activities and their related tasks, Risk Assessments, and client needs. This includes briefing staff / volunteers on clients who are new to group or who have had a change.

About you
***- Certificate IV in Leisure & Health or equivalent desirable.

  • Certificate III Aged Care or equivalent.
  • Experience in guiding staff to conduct programs that meet client needs, are safe and follow the program objectives.
  • Demonstrated working experience within an aged care/community health setting to develop, monitor and amend programs to create a stimulating and engaging program that within the funding scope.
  • Understanding and proven commitment to person centred care and client driven services.
  • Good interpersonal skills.
  • Current Victorian Driver's Licence and First Aid Certificate desirable.
