aged care, NDIS, early education, and family & community programs.


What you will be doing
In this role, our part-time NDIS Support Coordination Officer will provide case management support to NDIS participants in the following areas:

  • Support NDIS participants to implement their plan and increase their capacity to self-direct.
  • Support participants to make informed decisions about their care and lifestyle needs.
  • Support participants to engage with their local and broader communities.
  • Support participants and their carers to access formal and informal supports.
  • Establish and maintain referral pathways and professional networks to meet the needs of participants

  • Tertiary qualifications in Bachelor of Social Work or equivalent in the Human Services Field
  • Demonstrated minimum 2 years experience in similar role and knowledge of advocacy work
  • Awareness of the needs of people with disabilities and their carers
  • Proficient computer skills, with ability to learn and adapt to new software
  • Demonstrated proficient interpersonal skills
  • Able to drive to conduct client visits

Other requirements:

  • Current driver license and access to own vehicle
  • Undergo national police record check
  • Own IT infrastructure at home to enable WFH capabilities during periods of lockdown or where directed by the business
  • In accordance with PRONIA policy, a minimum 2 doses of the COVID19 vaccine or have a valid medical exemption certificate
