POSITION DESCRIPT
I
ON - Social Participation Coordinator

Purpose
The Social Participation Coordinator will be responsible for planning and supporting delivery of the
wellbeing initiatives that optimise on client strengths and maximising their capacity to remain
independent and engaged in community life. Strategic collaborating in the delivery of programs is key
component of the position and requires effective networking and collaboration skills.

Reporting to the_ _Team Leader Positive Ageing and Inclusion this position has a significant focus on
supporting key business and program initiatives such as Wellbeing and Resilience Program,
Commonwealth Home Support Programme (CHSP), Community Passenger Networks (CPN). The
position will plan, promote, deliver, monitor and review the delivery of the wellbeing programs, social
programs (group and individual), the meals service and transport programs.
The position will provide positive leadership within the Positive Ageing and Inclusion team, developing
and coaching staff within their roles and supporting programs and events.

Strong communication skills and ability to maintain solid relationships (with internal and external)
stakeholders will be vital to this role. As an ongoing commitment to the organisation you will be
responsible for delivering the best service to our community and to improve residents' quality life;
continuously, smartly and efficiently in the scope of your daily work.

Operational Leadership

  • Lead the operations of the community transport, meals and social support programs.
  • Promote and deliver strong customer experience to external clients and internal departments.
  • Coordinate the delivery of activities, programs and events that meet grant funding requirements, and
consider the needs of our diverse community.

  • Use effective evaluation measures and statistics for all programs and events and prepare reports in a
timely manner.

  • Interpret and ensure compliance with all relevant statutory obligations, Acts, Regulations, Aged Care
Quality Standards.

  • Develop and review policies, procedures, and work instructions with a focus on continuous
improvement, risk management, delivery of good customer experience and alignment with
Commonwealth Home Support Program (CHSP) Manual, Guidelines, and Good Practice Guide.
