Unit Profile

  • The Health Equity, Access and Care Coordination unit is focused on providing contemporary and evidencebased models to facilitate equitable and seamless access and care coordination to patients and families across Children's Health Queensland.
  • The unit brings together services across the continuum of care that facilitate access and/or support all patients and families of CHQ to effectively navigate their healthcare journey.
Services within the Health Equity, Access and Care Coordination unit include:


  • CHQ Connected Care Program
  • CHQ at Home (Hospital in the Home and Home Ventilation)
  • Navigate Your Health
  • Specialist Outpatient Access Team (CHQ Specialist Outpatients Contact Centre, Referral Room, Interpreter Services and Telehealth Services)
  • Indigenous Health Liaison Officers.
  • The unit is further focused on facilitating and coordinating equitable access and care for Aboriginal and Torres Strait Islander patients and families.
It is essential that the person who holds the position be an Aboriginal or Torres Strait Islander person.

It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 that applicants are Aboriginal or Torres Strait Islander peoples.

Your Opportunity

  • The position of Assistant Director of Nursing
  • Health Equity, Access and Care Coordination provides contemporary operational and professional nursing leadership for the services located within and connected to the unit.
  • The role shapes clinical and corporate governance; and provides the coordination of service delivery, human and material management, expertise in health care team communication and highlydeveloped data analysis and fiscal management to facilitate safe, quality, effective and efficient healthcare provision.
  • The position will partner with Divisional and Executive leadership across the organisation to support equitable and highquality care provision to Aboriginal and Torres Strait Islander patients and families.
