A little about the role


As an experienced Home Care Packages Care Manager, you will work collaboratively with the DPV Health Engagement Team - HCP & NDIS, Allied Health Services and the Social Support Program.

Your guidance and direction will assist our clients, their carers and representatives with the development and implementation of care plans that are flexible, personalised and culturally appropriate.

In this team, you;

  • Review intake information, engage new clients, establish home care agreements, care plans, budgets and connect our clients with local services and community resources
  • Conduct client visits practicing person centred care planning, provide independent advice to clients and their families enabling personal preferences and independence at home
  • Interact with DPV Health team members, stakeholders, partnered service providers representing the best interests of clients.
  • Conduct annual reviews for all clients revise care plans, goals, service provisions to quality standards and budget expenditure and maximisation
  • Ensure service provisions are conducted ethically, fairly and within the statutory legal and contractual requirements at all times
  • Work from Newbold House in Craigieburn

Desired Skills and Experience:


To be considered for this rewarding role you will have:

  • Extensive case management experience (2 years experience minimum)
  • Knowledge of & enthusiasm for Community Aged Care Services & Consumer Directed care environment
  • Demonstrated experience in working with ACAT, RAS, Care Navigators and other referrers
  • Strong experience in managing client budgets
  • Competence in managing complex clients
  • High level problem solving, exceptional customer service skills and analytical skills
  • Excellent IT skills, including Microsoft Office suite
  • Knowledge of Carelink +
  • Diploma level or above qualification in community, aged, health or another discipline desirable
  • Full Victorian Driver's Licence
  • Happy to travel within Hume & Whittlesea LGA's
  • Strong computer skills
  • COVID Vaccinations including booster
  • A clear national police check upon role offer
