About the role


We are seeking an Initial Assessment & Planning Worker - Metropolitan Melbourne Prisons position is to ensure high quality provision of housing, information and referral services to persons at risk of homelessness, within and exiting custodial settings in metropolitan Melbourne, as well as the Barwon region.

Responsibilities include

  • Assessment of immediate and longterm housing needs to service users who are at risk of homelessness exiting designated facilities
  • Assist to resolve existing social housing tenancy on entry to a period of custodial care
  • Explore service users to appropriate housing options including crisis accommodation
  • Assess requests where appropriate and financial assistance from the Housing Establishment Fund (HEF)
  • Attend prisons to interview eligible individuals identified through referral system
  • Provide occasional service provision at other custodial facilities managed by the Housing Pathways Program as required


This is a fixed term (til June 2024) full-time position, based at the HPP office, situated at the Western Metro Homelessness Service in Sunshine, VIC.

Salary and conditions are in accordance with SCHADS Level 5.

What you will have

  • Demonstrates knowledge of homelessness including context and legislation
  • Understands homelessness practices when dealing with clients
  • Ability to understand the needs of individuals experiencing homelessness
  • Tertiary qualification in Social Work or related Community Service field favourably regarded
  • Knowledge of Justice system and experience working within correctional settings highly desirable
  • National Police Record Check required
  • Current and valid Working with Children's Check
  • Current VIC Drivers Licence
  • Ability to lift 5kg and carry items short distances due to instances where you may be required to take donated items to clients
