Nursing Team Leader - Talk To You Tomorrow

Temporary (3 year position - until end of June 2026) part time role

Currently based at Maroondah Hospital, however, this could be flexible

$107962 P.A. (paid pro rata)

About the role

The Nursing Team Leader of Talk to You Tomorrow (TTYT) team is a valued and collaborative member of the interdisciplinary health care team at Eastern Health, and is accountable for managing, coordinating and delivering safe and comprehensive patient centred care, while working competently within their scope of practice, and in alignment with the Eastern Health and Nursing & Midwifery Practice Standards

The TTYT Team is an innovative post-discharge clinical review service, consisting of medical, pharmacy, nursing and clerical team members.

The TTYT Nursing Team Leader will manage the day to day nursing and clerical team within the TTYT team, and will work collaboratively with the other multi-disciplinary team members to provide telephone, telehealth or face-to-face review, in the general medical population, in the first 7 days post-discharge from the acute hospitals.

The intervention is aimed to prevent readmission and identify referrals needed to keep the clients safely at home.

This is an opportunity to allow General Medical patients to be discharged home in a more timely manner, confident in the knowledge that they will be provided timely follow up from a multi-disciplinary team

About you


To be successful in this role you will possess:
Registration with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA

At least 5 years relevant and recent clinical experience within a team leader role or similar

Post Graduate qualifications desirable or working towards

Current Victorian Driver's Licence

Highly developed interpersonal and leadership experience

A strong commitment to high quality care

The ability to work as part of a diverse team

Excellent communication and organisational skills

Vaccination against infectious disease is a mandatory requirement of this role.
