About the role:


The Client Intake and Support Officer works within the Mission and Mission in Action of HammondCare to ensure that administration services are tailored to the needs of individual clients and maximise client satisfaction.

The role undertakes a key role in the Restorative Care frontline team, providing reliable office-based support for allied health staff, clients, and their representatives.


Essence of the role:


  • Provide reliable officebased support for the allied health team
  • Ensure efficient triage (escalation) of enquiries and referrals
  • Work closely with Care Coordinators to ensure effective program administration, including ordering and tracking of equipment for clients
  • Provide administrative support to facilitate allied health meetings and case conferences, including distribution of agendas and production of minutes
  • Responsible for client documentation from referral to commencement of the program
  • Provide system and staff administration by maintaining uptodate data on allied health staff information
  • Participate as a member of the Restorative Care team in a continuous quality improvement team environment

About you:

Working knowledge of ComCare and My Aged Care portal, aged care support packages such as Commonwealth Home Support Package, Restorative Care programs and Short-Term Restorative Care programs is highly desirable:

  • Demonstrated experience in customer service, and commitment to excellence to ensure the consistent provision of exceptional service to internal and external stakeholders
  • Demonstrated ability to use initiative and problemsolve with limited supervision.
  • Demonstrated ability to record, report and maintain information
  • Strong administrative and computer skills
  • Ability to work independently and as part of a team
  • Empathy and patience.
