Global Contractor Management Solutions is looking for a skilled Disabilities Services Officer who fits the following profile:Key Responsibilities:
Responsible for providing disability support, meeting individual client needs in their home or community environment to enhance independence, abilities, community participation and quality of life.
Tasks and Responsibilities:
- Assessing clients' needs; provide direct support to people with a disability, on a daytoday basis, according to individual needs, plans, programs and direct observation.
- Developing and implementing support programs; communicate and interact with consumers, families, community agencies and staff on day to day and other matters as necessary.
- Interviewing clients and assessing the nature and extent of difficulties
- Administration of necessary medicines, medical assistance and maintenance of health records
- Monitoring and reporting on the progress of clients: maintain consumer and home records, write notes and routine reports
- Assessing community need and resources for health, welfare, housing, employment, training, and other facilities and services
- Communicating with community groups, welfare agencies, government bodies and private businesses about community
- Issues and promoting awareness of community resources and services
- Assist people with disabilities to experience a betterment of life and enhancing their independent living skills.
Skills & Experience
- Tertiary Qualifications in the Health Care/Aged Care industry
- Over 2 years of work experience in the industry
- An understanding of the ageing process
- A comprehensive clinical knowledge, particularly to age related health issues
- Be reliable, genuine, compassionate, caring and positive in demeanour
- Understanding of and commitment to continuous quality improvements
- Excellent communication and interpersonal skills including demonstrated experience interacting with a wide range of people
- Ability to prioritise and manage workloads according to residents needs
- Ability to work as a member of a multidisciplinary team
Salary:
$65, $75,000.00 per year
