Experienced
NDIS Worker Screening Check (or current valid Yellow Card)
Working with Children Check (Blue Card)
Current driver's license and reliable vehicle
Must be living in Australia (the position is in Melbourne Vic)

Providers to assist them in understanding, implementing, and coordinating their NDIS Plan. Through participant-focused collaboration and coordination, we ensure that our participants have choice, control, and independence with their plans.

About the business

  • Total Care Foundation looking for level 2 Support Coordination NDIS participants across the Gold Brisbane. As the main office is located in Melbourne.

  • Manage a portfolio of participants and collaborate with the team
  • Review and assess the budget, funding, and services available to participants
  • Coordinate and connect NDIS participants with relevant support providers
  • Support participants to understand their NDIS plans, goals, and objectives and use the NDIS portal
  • Assist participants to understand their rights and build their confidence so that they can make informed decisions
  • Assist participants to strengthen their capacity to establish networks, maintain support services, linkages, and relationships
  • Build rapport and strong working relationships with participants and stakeholders
  • Strive for solutions that proactively reduce participant's barriers and risks
  • Complete all required reporting in a timely manner
  • Proactively manage and update key participant information and documentation where required
  • Implement strategies to ensure seamless communications across all networks
  • Work with external service providers & suppliers to ensure the best outcome for participants
  • Develop and maintain quality working relationships with each participant and their stakeholders

Skills and experience

  • Demonstrated ability to work in a personcentered environment to build capacity, choice, and control
  • Ability to build and maintain rapport with a diverse range of customers and stakeholders, including people living with a disability, cultural diversity, and language other than English
  • Prior experience in the disability sector
  • Relevant qualifications in the disability sector
  • Have experience and/or knowledge of the National Disability Insurance Scheme (NDIS)
  • Educated in the NDIS price catalogue, NDIS plan budgets, and participants' pathways
  • Strong knowledge of mainstream services and community support, and experience in community networking
  • Ability to develop effective working relationships with team members, people with disabilities, their families, and stakeholders
  • Have experience in coordinating personcentered supports
  • Demonstrate problemsolving skills using a logical and efficient approach
  • Ability to work independently and as a collaborative team member
  • Excellent written and verbal communication
  • Technologically savvy
  • Outstanding time management skills for management of plan assessment, reviewing, and reporting
  • Proven ability to meet administration, report writing, and compliance obligations.
  • Understanding of the NDIS including an understanding of the rules for reasonable and necessary supports
  • Experience with a diverse range of disabilities
  • Experience in support coordination, and/or community services
  • Ability to advocate on behalf of participants and a commitment to participant's choice and control

  • Working with Children Check (Blue Card)
  • COVID Vaccination Certificate (or a Valid exemption)
  • Current driver's license and reliable vehicle
  • NDIS Worker Orientation Certificate.
  • Must be living in Australia (the position is in Melbourne Vic)
