Exciting opportunity for an experienced or aspiring case manager to partner with our Vulnerable Aged Care clients who are at risk of homelessness, have experienced homelessness or are impacted by Hoarding and/or Squalor


As part of the Vulnerable Persons team, you will use your skills and experience to ensure that all clients across Sydney Inner City and South East Sydney have a flexible, responsive support service/intervention tailored to their individual needs.


  • Your role will be_:
  • Assertive outreach by engaging and building rapport with potential clients and local intermediaries, including outreach activities to target groups and target locations.
  • Assess, Coordinate and review client care needs within their funding allocation.
  • Coordinate a service response that is directed to ensuring housing is maintained for the older person and that their care needs are met so they can continue to live in the community
  • Utilise your extensive knowledge of the aged care sector to help your clients to navigate the system
  • Collaborate with both government and nongovernmental support agencies within the community care sector to achieve improved housing, health, and wellbeing outcomes for clients.
  • Partnerships are developed and maintained with key professionals to support the identification and engagement of the target groups.
  • Have a genuine passion for care, balanced with a desire to maximise services and offer commercially sustainable case management in line with the organisation's strategy.
  • To be successful in this role:_
  • High level of communication skills and a "no judgement" approach
  • Understanding of the complexities associated with situations of Hoarding and/or Squalor desirable but not essential
  • Relevant qualifications in Allied Health, Nursing, Social Science or related field highly regarded
  • Highly developed planning, influencing and negotiation skills
  • Demonstrated experience in managing the contractual requirements of government programs to ensure compliance, regulatory standards, and organisational policy and procedures are followed
  • High degree of computer literacy with an ability to learn new software programs including Orca & Procura
  • Registered and insured motor vehicle and a current unrestricted Australian drivers' licence are essential
