About the Role

Deivian Care is looking for a Service Coordinator in Indooroopilly (QLD). The primary purpose of this position is to assist the Operations Manager in leading a team of Support Workers to deliver quality, person-centered and customer focused outcomes for the people we support. Within the parameters of the role, the position is responsible for the quality of services, ensuring timeliness, and working within the agreed budget. The role is also responsible for implementing a planned approach to the leadership, supervision and professional development of the team members. The primary function of this role is to support stakeholders (clients, client families, and employees) to achieve their individual goals and objectives in accordance with their participants Individual Care Plan.

Job Role and Responsibilities

  • To ensure support workers are rostered correctly.
  • To ensure quality service to clients and their families.
  • To contribute to the implementation of the Mission, Values, and Vision of Deivian.
  • To assist clients in activities of daily living, personal care, social activities, and community access through the implementation of individual care plans, and programs that are age appropriate and relevant to individual goals.
  • To ensure all Deivian's policies and procedures are adhered to.
  • To demonstrate respect, understanding and client dignity.
  • Conform with the duty of care requirements ensuring workplace health and safety requirements are adhered to.
  • Must not misuse items or facilities provided in the interest of safety and health.
  • To demonstrate confidentiality in all matters relating to services provided to clients and service information generally.
  • To play an active role in the planning, design implementation and evaluation of daily programs.
  • To update client files and document evaluations.
  • Assist clients to take medications and maintain client medication folders as required.
  • Attend staff meetings and training sessions as required.
  • Accurately record in daily communication books, client files and medication folders.

Experience/Certification Required

  • Bachelor's degree or relevant certification in aged care or disability
  • Full Queensland Driver's License and your own vehicle (registered in your name)
  • National Criminal Check
  • Working with Children Check
  • Covid19 Vaccination Certificate

$ $32.00 per hour + Super
