ABOUT THE ROLE


You will be responsible for the provision of comprehensive and culturally and linguistically appropriate case management services for Spanish speaking elders.


You will competently assess the physical, social, and psychological needs of HCPP clients and develop, implement and routinely monitor individualised care plans and budgets in line with the Aged Care Quality standards and compliance obligations.


You will implement best practice and high quality case management interventions within the scope of HCPP guidelines and aged care legislation.


SELECTION CRITERIA

QUALIFICATIONS_

  • Bachelors Degree in Social Work or Social Sciences, or related discipline, or Diploma in Community Services or related field.

EXPERIENCE_

  • Comprehensive understanding and experience in a Community setting or in a Case Management Role.
  • Experience in the provision of HCPP and CHSP services is desirable but not essential
  • Experience working with a Client Management software package is preferred, but not essential

KNOWLEDGE_

  • Sound understanding of Home Care Packages (HCPP) and Commonwealth Home Support Program (CHSP) guidelines and relevant legislation
  • Understanding of Aged Care Quality and Safety standards, and of personcentred philosophy and Consumer Directed Care (CDC).
  • Thorough understanding of Wellness and Reablement philosophy.

  • Ability to plan and coordinate services according to relevant program guidelines.
  • Sound assessment and care planning skills.
  • Problemsolving, decision making and organisational skills.
  • Ability to promote transparency and quality service delivery, achieving high levels of client satisfaction.
  • Ability to develop and maintain links and networks with specialist services.
  • Strong stakeholder management, influencing, and relationshipbuilding skills.
  • Ability to communicate effectively with clients with different needs, such as people from culturally and linguistically diverse (CALD) and LGBTI backgrounds, or people with a disability.
  • Ability to liaise and communicate effectively with a broad range of people, both internal and external to the organisation, in a professional and ethical manner.
  • Ability to promote the organisation's policies and practices focused on building a diverse workplace.
  • High level administrative and organisational skills.
  • Excellent computer literacy skills in spreadsheets, databases, word processing programs and software

E-tools, (or similar Client Management System), Services Australia Aged Care Provider Portal (ACPP), and My Aged Care (MAC) portal.


  • Fluent in Spanish and English and have a sound understanding of the Spanish speaking communities' history, culture, and needs.

  • Able to commit to 15 hours over 23 days per week (Mon

  • Victorian Drivers Licence and reliable, insured car.
  • COVID VACCINATIONS up to date.
