About You

Experienced in supporting the needs of people with complex disability and/or mental health you will be motivated by seeing these individuals thrive within their communities as a result of care plans that promote independence, resilience and confidence.

These skills will have been gained in a role with similar case management responsibilities where you have supported a portfolio of clients in helping them to access supports and services within their community.


Additional attributes we are looking for include:

  • An understanding of NDIS Framework and Support Coordination
  • Knowledge of accessing community resources, particularly in the areas of health, drugs and alcohol, legal, income support, employment and recreation
  • Passion and commitment with a flexible approach
  • Effective written and verbal communication skills
  • Emotional resilience, patience and determination to get the job done


We are looking for someone that will take the initiative to ask questions and can work collaboratively, being comfortable that this will be a work from home arrangement.

You will have a willingness to learn and ability to stay motivated while working autonomously.

About the Role


As a Support Coordinator for ermha365 you will actively manage a portfolio of clients and provide coordination in accordance to their NDIS plan, assisting to connect with supports, build capacity and resilience.

We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes.


To achieve this the role entails:

  • Conducting needs assessments based on their NDIS plan and goal
  • Develop a support plan that identifies clear actions
  • Enabling the client to connect with various service providers
  • Assist clients to achieve goals through building capacity, knowledge, resourcefulness and confidence
  • Provide support to the families of clients
  • Work with local service providers and community members
  • Build and foster positive ongoing stakeholder relations
  • Prepare case notes, reports and updates as required by ERMHA365 and the NDIA


It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver's licence.
