Care Fast is looking for a part-time or full-time Support Coordinator to commence in January 2023.

The position is primarily focused in South East Melbourne but will require the person to travel to other locations within the region.

We are looking for someone with a passion for relationship building and supporting participants.

In this role you will, and not limited to:


  • Research, coordinate and manage a range of supports to suit individual needs
  • Supports participants to understand their NDIS plan and plan funding
  • Assisting people in managing their supports and services to enable great outcomes and achievement of NDIS Plan goals
  • Supporting people with the NDIS planning process and reviews
  • Undertaking risk assessment and support planning processes with people Engages with participants and their informal support networks and builds connections with providers, local community services and NDIS contacts
  • Support participants to strengthen their ability to connect with informal, mainstream and funded supports, and to increase capacity to maintain support relationships, and resolve service delivery issues within their local communities
  • Gathers and prepares evidence for the participant for plan reviews
  • Develops/has a researchbased approach for understanding the NDIS and sourcing available supports
  • Perform all underlying administrative tasks related to support coordination service provision including budgets

Skills and experience:


  • Minimum 1 year Support Coordination or Local Area Coordination experience
  • Outstanding knowledge and experience of NDIS and their processes
  • Strong interpersonal, written, and verbal communication skills including listening
  • Ability to develop good working relationships with internal and external stakeholders, including participants, families, and health providers
  • Effective time management, caseload management, problemsolving skills, and ability to plan and manage resources/budgets
  • Able to coordinate service expectations and professional boundaries
  • You can coordinate, organise and track activities/bookings/tasks
  • Able to selfmanage to achieve multiple and often competing deadlines
  • Strong administration skills including basic budgeting
  • Experience in using Microsoft Office suite of products
  • Experience in mental health would be well regarded

An applicant MUST have:


  • Minimum Certificate IV in Disability or related field
  • NDIS Worker Screening Check
  • Working with Children Check
  • Proof of Vaccination Status
  • Current First Aid and CPR Certificate
  • Australian Driver's license and vehicle

Salary:
$58, $85,000.00 per year
