Blossom Community Care is a registered NDIS provider. We provide services ranging from Support Coordination, Plan Management, Support Workers, Allied Health Services, and Cleaning and Gardening Services. We aim to create a fully supported lifestyle by providing awareness and care.

We started with a small 1 room setup providing caregiving services to patients.

Over many years, we have broadly expanded and are providing health, personal care, and many more services to patients and the elderly in Victoria.


About You


The
Blossom Community Care family is growing, and we are seeking a motivated and enthusiastic Support Coordinator to join our dedicated team.

You will enjoy working with all age groups, and managing a varied caseload and are passionate about providing holistic services to participants to create meaningful outcomes, helping them live life to their full potential.

Care Coordinator responsibilities include:


  • Assessing the service needs of clients and linking them to the appropriate resources and providers
  • Facilitating programs and services, job training and transportation
  • Developing a resource directory of local social service agencies and providers for easy access

Job brief

  • We are looking for a Care Coordinator to join our team to provide recommendations and suggestions for new services to our clients. We need someone detailoriented with an eye on customer satisfaction to ensure clients receive the services they need for a speedy recovery.
  • A Care Coordinator's responsibilities include working directly with clients to ensure their care. They explain what options are available and keep records about everything offered in the facility.

Responsibilities

  • Advocate for adequate, timely and costeffective services
  • Respond to any issues that occur during the delivery of services
  • Assist clients with living independently in their own homes
  • Schedule appointments for clients, answer phones and track clients' service records
  • Monitor the services being provided and stay up to date on any services being introduced or discontinued
  • Evaluate the quality of all services and identify areas that need improvements

Requirements and skills

  • Proven work experience as a Care Coordinator or similar role
  • Strong work ethic and service skills
  • Excellent communication and interpersonal skills
  • Stay up to date with services, policies and regulations
  • Strong recordkeeping and analytical skills
  • Excellent organizational skills and attention to detail

Knowledge & experience:


  • A clear understanding of Goal and Objective setting in relation to identified Participant needs.
  • Experience in dealing and communicating with people with disability or NDIS participants.
  • Experience in identifying, developing, and evaluating communication matters.
