About the Opportunity
Campbelltown Community Health Centre is seeking a highly skilled, passionate and dynamic Social Worker to join their dedicated team.


Bringing your extensive clinical and leadership experience to the role, you will be able to maintain a calm and professional manner.

Within the role you will be dealing with highly complex & challenging cases and be working in a fast paced exciting environment.

We are seeking a strong communicator who is approachable, adaptable and looking to step into a leadership role.

You will have the chance to complete policy reviews, write reports and assist the team leader in their duties to provide you with a great opportunity for learning.

You will be managing a diverse caseload whilst also coordinating the team.


What You'll be Doing

This position is responsible for providing specialist Social Work service to consumers and their carers particularly in the Neurodegenerative Disease Supportive Care Service to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.


Where You'll Be Working

South Western Sydney Local Health District has implemented a newly established specialist service to enhance community care for people with late-stage neurodegenerative and chronic conditions and disability across she District.


The team will provide support for the community and is a multidisciplinary service which aims to provide focused care with access to Physiotherapists, Speech Pathologists, Occupational Therapists and specialised nursing staff.

The Neurodegenerative Disease Support Care program will ensure quality, patient centered care for those suffering a late-stage neurodegenerative disease and will strengthen the relationships with specialised palliative and community care services.


Working with a highly skilled, multidisciplinary team will provide a range of opportunities for growth and development and will provide a diverse patient base for all clinicians to ensure learning throughout your career.


How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible.

  • A Social Work degree and eligibility for the Australian Association of Social Workers (AASW) with extensive post graduate experience working as a Social Worker in relevant clinical areas in a health setting, including a demonstrated understanding of complex health conditions and the impact on wellbeing.
  • Demonstrated ability to lead complex casework including communication and collaboration with the patient and other stakeholders in order to maximize positive patient outcomes and collaborative partnerships
  • Demonstrated counselling skills to address clientdistress, and ability to provide counselling / therapies that focus on the psychological, behaviour, relational or social causes of distress
  • Experience working independently and collaboratively within a multidisciplinary team.
  • Demonstrated high level communication skills (written and verbal), computer and prioritisation skills
  • Demonstrated experience in the provision of clinical supervision and expert advice/consultation to the social work service and the clinical specialty.
