Your New Role- Provide social work service across the rehabilitation team including stroke and amputee rehab, including assessment, intervention and discharge planning for patients in the sub-acute hospital.
- Liaise with service providers and NDIS pathways, Community Services, Aged Care Assessment Service and Transition Care Program.
- Support and guide early career Social Workers through supervision.
- You will be well supported by Senior Social Workers on site, Social Work Clinical Lead (Grade 4), Social Work Manager, and the Social Work team.
- You have excellent communication skills and a passion for continued professional development for yourself and others.
- You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers with at least 4 years' experience.
- You value working with supportive and dedicated health professionals and multidisciplinary teams.
