Your New Role- Provide social work service across the rehabilitation team including stroke and amputee rehab, including assessment, intervention and discharge planning for patients in the sub-acute hospital.

Liaise with service providers and NDIS pathways, Community Services, Aged Care Assessment Service and Transition Care Program.
Support and guide early career Social Workers through supervision.
You will be well supported by Senior Social Workers on site, Social Work Clinical Lead (Grade 4), Social Work Manager, and the Social Work team.

About You- You have demonstrated hospital experience with a collaborative working style

You have excellent communication skills and a passion for continued professional development for yourself and others.
You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers with at least 4 years' experience.
You value working with supportive and dedicated health professionals and multidisciplinary teams.
