Permanent Part Time, 3 days a week (Monday to Friday)

At up to three residential sites, in Eastern Health catchment area

About the role

The Transition Care Program provides care and restorative services for older people following a hospital stay.

Through low level goal oriented therapy clients are supported to reach their functional and lifestyle goals and plan for longer care options.

The bed-based program is provided at three residential facilities, currently located in nearby suburbs to Wantirna Health.

The program is a time-limited program and Allied Health Assistants play a key role in individual and group activities to enhance the experience of clients.

About you


To be successful in this role you will possess:
Demonstrated interest in and experience working with older people

A human rights-based approach to all aspects of work

Commitment to provide safe and effective care

Willingness to reflect on self and engage in learning

Proficiency in individual therapy, group therapy, equipment and environment management and communication of patient information

The ability to work as part of a diverse team

Sound judgement and understanding of professional boundaries

Completion of Certificate IV (Allied Health Assistant) or recognised equivalent, and 80 hours minimum clinical experience.

Vaccination against infectious disease is a mandatory requirement of this role.

Selection Criteria


Essential:

  • Completion of Certificate IV (Allied Health Assistant) from a registered training organisation, or recognised equivalent inclusive of relevant discipline modules to advertised role.

Essential:

  • Completed a minimum of 80 hours of clinical experience
