About the Role

You are passionate about improving safe, equitable access to health care for people with disability.

You will work directly with people with disability supporting them, their families and carers to access health services across the continuum of care at SVHM.

You will undertake targeted outreach for people with disability who may be less likely to use health services.

You will be supported by your clinical team to identify and resolve barriers to accessing health care services at SVHM for people with disability- Support people with disability, their families and carers to safely access appropriate health services including COVID19 vaccination, assessment and treatment_- Undertake outreach where there is evidence that people with disability are not using health services because of fear of exposure and infection with COVID19, or uncertainty about how the service will operate_- Undertake targeted outreach for vulnerable people with disability who may be less likely to use health services (e.g., Aboriginal people with disability, LGBTIQ people with disability etc.).

_- Provide support, advice and education to SVHM treating units; contributing to patient well-being and safety, service accessibility, discharge planning and continuity of care needs for patients with disability_

Applicants with lived experience of disability will be highly regarded, particularly those with experience or understanding of intersectional identifiers

Classifications for this position will range from Grade 2 Year 1-4 Allied Health Clinician and/or Grade 3B Registered Nurse:
Occupational Therapist Grade 2 VF- VF9 ($83,361 - $95,825 per annum, pro rata)

Social Worker Grade 2 SC2- SC24 ($83,361 - $95,825 per annum, pro rata)

Physiotherapist Grade 2 VB- VB4 ($83,361 - $95,825 per annum, pro rata)

Grade 3B Registered Nurse YU11 ($99,299, per annum, pro rata)

Your Contribution- Tertiary qualification in a relevant course of study- Current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant registration body- Minimum of two (2) years' post-graduate experience, in a relevant field of disability and /or community based service and /or field of acute/subacute/rehabilitation health care- Demonstrated experience working in a health setting with people in vulnerable or marginalised patient populations; disability, complex and chronic health and social issues- Current National criminal history check

About Health Independence Programs - Pain and Disability Stream


The Health Independence Programs (HIP) consists of multidisciplinary outpatient rehabilitation programs, complex care services and specialist clinics operating across various campuses of St Vincent's Hospital Melbourne.

The Disability Liaison Officer Program is part of the Disability and Pain Stream, based at St Vincent's Fitzroy. The service offers a supportive and dynamic team environment that focusses on patient-centred care.
